GAD-7 Anxiety Quiz
The Generalized Anxiety Disorder-7 (GAD-7) is a questionnaire very similar to the PHQ-9, but it consists of seven questions assessing for anxiety rather than depression. It is now frequently used in clinical settings to evaluate the possibility of a patient having a generalized anxiety disorder.
Y-BOCS OCD Quiz
The Yale-Brown Obsessive Compulsive Scale (Y-BOCS) is considered the “gold standard” measure of obsessive-compulsive disorder (OCD). Taking the Y-BOCS helps healthcare providers assess a patient’s obsessions and compulsions, and if their score is high, the provider will continue to ask the patient to specify what their obsessions and compulsions are about.
MDQ Anxiety Quiz
The Mood Disorder Questionnaire (MDQ) assesses the symptoms of mania, which, in addition to depressive symptoms, is a feature of bipolar disorder. The MDQ consists of 13 yes-or-no questions about manic symptoms, such as energy level, irritability, excessive self-confidence, and risky behaviors, and asks if these symptoms occurred at the same time and how much they interfered with a patient’s daily life.

The Alcohol Use Disorders Identification Test (AUDIT-C) is an alcohol screen that can help identify patients who are hazardous drinkers or have active alcohol use disorders (including alcohol abuse or dependence).
Drug Abuse Screening Test
The Drug Abuse Screen Test (DAST-10) was designed to provide a brief, self-report instrument for population screening, clinical case finding and treatment evaluation research. It can be used with adults and older youth.
